Once it’s activated, you can’t patch yourself back up to full health and continue to reap the benefits. You have to stay at or below 35% health to have this active.

This buff is best-utilized solo, to prevent wasting gem or equipment slots. You should also dismiss your Palico to prevent periodic auto-heals.
